I, (Person) , (Position) certify that I am authorized to act alone to establish a Website Owners Club membership with TWClark LLC as requested on this application returned with this electronic mail message ("Application") on behalf of (Company Name) . I further certify that the electronic address used to transmit the Application represents the business' electronic mail address. If the application is from an entity other than a sole proprietor, I represent that any and all required actions, such as corporate resolution, partnership agreement, or governmental authority have been completed and are in full force and effect and further that the act of submitting the Application, entering into and maintaining a Membership and other relationship(s) with TWClark LLC as requested on the Application is within my authority and within the business' power.

By sending this electronic message to TWClark LLC, as applicable, I,

1. confirm that (Company) is applying for a (Class) Membership in the Website Owner’s Club
2. agree that TWClark LLC may charge my credit/debit card a one-time Membership Fee of ($x.xx) and I understand if this is a discounted fee and if I terminate my membership within two years that I will pay the difference of the full Membership Fee and this amount or for services rendered whichever is less.
3. understand that Membership Fees are nonrefundable after 30 days.
4. agree that TWClark LLC may charge my credit/debit card my Monthly Dues of ($x.xx)
5. understand that my Monthly Dues may/will change
   • On my anniversary (May increase, decrease or stay the same)
   • When my class changes (May increase or decrease)
   • When my utilization exceeds 200% in three or more months or 500% in any year.
6. understand that (Company) is entitled to all unlimited services as are available in the Class of Membership for which I’m applying
7. understand that I will receive an unlimited, non-exclusive license to all code and products created for me
8. understand that I will receive licensed rights for all 3rd party products
9. understand that I may terminate my fully paid membership at any time without penalty
10. understand that other Support Agreements that I may have with TWClark LLC are terminated

I agree that sending this electronic message to TWClark LLC has the same effect as a written signature on a copy of this Application and the Document of Understanding located on the TWClark LLC website.

I further represent that I am capable of printing copies and will maintain record copies in written or electronic form.
